Jacksonville Medical Examiner mistake leads to wrongful cremation mistake

cemAn autopsy technician misread a number on a body tag, leaving one family without a body to memorialize and another with the wrong person’s ashes scattered across the Atlantic Ocean.
As a result of the mistaken identification, the technician released the body of John Chappell, 36, instead of that of Judith Perez, 65, said Duval County Chief Medical Examiner Margarita Arruza.

“It was off by one digit. He released the wrong body to the funeral home, and obviously the funeral home never checked or looked at the body,” she said. She said the technician is no longer employed by her office.

Perez’s family had the body cremated and scattered the ashes in the ocean before the mistake was discovered when Chappell’s relatives showed up to claim his body.

Chappell’s brother in Jacksonville and sister in Mt. Morris, Mich., have notified the city of their intent to sue for negligence, said their cremation attorney Henry Gare.

“It is a very heartbreaking situation,” Gare said Wednesday.

Gare said the family had planned to scatter Chappell’s ashes at his grandfather’s grave in Michigan.

A woman who answered the telephone at Green Pine Funeral Home in Yulee and refused to give her name said Chappell’s body was received in a bag identified as Perez’s remains. Because the body was cremated, she said there was no reason to open the bag.

Jacksonville Law Enforcement join forces for “click it or ticket Florida” initiative

The Jacksonville Sheriff’s Office along with the Atlantic Beach Police Department and the Florida Highway Patrol will have a zero tolerance policy towards unbuckled motorists from May 21 to June 3, 2007

Research from the National Highway Traffic Safety Administration show that just 80 % of drivers and passengers in Florida are using their safety belts.

The “Click It or Ticket Florida” initiative is a Jacksonville and Florida campaign to educate drivers about seat belt safety.

We cannot say how strongly we feel about drivers buckling up. Even if an adult chooses not to use a seat belt, there is no excuse for not buckling up our kids. We hope all parents and care givers take the time to protect their children by putting on their seat belts.

US Post Office Promotes Dog Bite Prevention Week in Jacksonville

Last year more than 4.7 million people were bitten by dogs in our country. Our Jacksonville Post Office along with the CDC are working together to help prevent dog bites. The third week of May is National Dog Bite Prevention Week. Every year 800,000 of these bites are bad enough to need medical attention. Half of those bitten are kids. Our office has seen a rise in dog bites to children under the age of 4.

We are also unfortunately seeing many more homeowner’s insurance policies with dog bite exclusions. I urge anyone who has a dog to see if their policy covers dog bites.

I personally have spoken to Jacksonville school assemblies and pediatricians about ways to prevent dog bites.

Children should be taught the following when it comes to dealing with strange dogs:

Do not approach an unfamiliar dog.
Do not run from a dog and scream.
Remain motionless (e.g., “be still like a tree”) when approached by an unfamiliar dog.
If knocked over by a dog, roll into a ball and lie still (e.g., “be still like a log”).
Do not play with a dog unless supervised by an adult.
Immediately report stray dogs or dogs displaying unusual behavior to an adult.
Avoid direct eye contact with a dog.
Do not disturb a dog who is sleeping, eating, or caring for puppies.
Do not pet a dog without allowing it to see and sniff you first.
If bitten, immediately report the bite to an adult.
